Job Description:
This is a 1099 Independent Contractor position.
Independent lead generation is required.
Prospecting and lead generation are your responsibility.
This is vital for promoting a Medicare Home Health and Home Palliative/Hospice service to ensure senior citizens are receiving the highest quality home care at NO cost.
SCOPE OF ENROLLEMT RESPONSIBILITY:
Referral Submission
Educate the individual on the benefits of the program. If they are interested:
• Complete a brief intake form.
• Submit the form to a email or portal provided along with the patients ID, Medicare card and any
other insurance card so the doctor assigned to the registration can validate and approve
the patient.
At this point, we are done. SIMPLE !!!!!
We are only waiting for the patient to be visited by the Doctor on staff to
confirm the validity of the registration form submitted and the general health of the patient.
Once completed, a nurse will be assigned to visit the patient to complete the agreement for
either home health or hospice care. At that point the referral is now approved and payable.

We offer a competitive payment of $600.00 - $700.00 per accepted home hospice referral.
