Are you on Medi-Cal and feel like you’re not as healthy as you should be… or you just haven’t been keeping up with check-ups?

You may qualify for a free wellness navigation program that gives you your own Health Navigator (LCM) — someone who helps you understand your health numbers, make a simple plan with your doctor, and check in once a month so you don’t have to do it alone.

💬 What We Do Together

Your Health Navigator can help you:

• Make or update a primary care doctor (PCP) visit
• Get a simple Wellness Snapshot from your doctor (blood pressure, sugar, weight, conditions)
• Turn your doctor’s advice into a clear, realistic health plan
• Check in with you about once a month to talk about what’s working and what’s hard
• Share ideas and local resources that support your health (food, stress, movement, etc.)

This is real, one-on-one support — not a call center.
You stay in charge of your health. We’re here to guide, explain, and encourage.

❤️ Who May Qualify

If you have Medi-Cal with one of these plans, there’s a good chance you qualify:

• Molina

• San Francisco Health Plan
• Santa Clara Family Health Plan

You may be eligible if any of these sound like you:

• You don’t know your blood pressure, sugar, or main health numbers

• You’ve used urgent care or the ER because regular appointments are hard to get
