Duties for this position include:

• Provide high quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the LA County Department of Health Services (DHS) Housing for Health (HFH) program
• Develop and coordinate individual client care plans for clients, to include goals, objectives and actions that address and resolve barriers
• Advocate and liaise to ensure the utilization of resources and quality outcomes
• Conduct intakes and enrollment activities to establish program eligibility, individual patient needs and goals
• Assess self-management skills of individual patients; promote and aid patient in establishing self-management skills
• Coordinate housing placement and individualized case management services to ensure housing stability
• Assist clients in obtaining income and/or establishing benefits, linkage to primary care and establishment of a medical home, locating employment and/or educational opportunities, obtaining basic needs, such as clothing and food, budgeting and money management, life skills and community participation and building the skills and capacity needed to successfully navigate healthcare and social service systems
• Maintains up-to-date and accurate records, client tracking data, and monthly reports, including appointments, services, follow-ups and assessments in multiple databases
• Attend required trainings, meetings and presentations at other agencies as required
• Attend staff meetings, case conferences, team meetings and trainings as required or assigned
• Work with medical providers to determine health priorities and develop appropriate care management plans
• Complete reports and file audits as mandated
• Other duties may be assigned as business needs dictate

• Working knowledge of and experience engaging and assisting individuals with chronic/complex medical conditions, such as HIV, HCV, SMI, SUD, to access/navigate health care as well as other needed services;
• Experience with Housing First and Harm-Reduction strategies, Motivational Interviewing, Patient Centered and Trauma Informed Care, Crisis Intervention & Prevention strategies;
• One year experience in social/medical services setting working with homeless individuals AND have a social work/mental health related Bachelors’ degree, OR
• At least two years of experience providing direct mental health or intensive case management services
• Efficient in Microsoft Office Suite
• High School Diploma (Required)
• CPR/First Aid Certified
• Degree in Human Services (Preferred but not required)
• Current California RN License or LNV License (Preferred but not required)
• Ability to lift 25lbs
• Bilingual English/Spanish highly preferred (read, write, speak)
• This position has no supervisory responsibilities
• Personal transportation is required
