Position Summary
We are seeking a skilled, compassionate Licensed Vocational Nurse (LVN) to work in our office supporting our clinical and administrative operations. This role is ideal for an LVN who wants to use their nursing expertise in a structured office setting — supporting patient care coordination, reviewing clinical documentation, and assisting with compliance and quality assurance.
Key Responsibilities
Assist with patient intake and coordination — verify insurance, collect medical history, ensure accurate and timely documentation.
Review, prepare, and track Plan of Care (POC), visit notes, OASIS, and other clinical documentation to ensure accuracy and compliance with Medicare and agency standards.
Communicate with field nurses, therapists, physicians, and patients regarding orders, medication changes, and follow-ups.
Help manage authorizations, orders, and renewals to support timely patient care.
Maintain and update patient charts in the EMR system (e.g., Kinnser, Axxess, HomeCare HomeBase).
Provide patient/family education and support by phone when needed.
Assist with quality improvement projects, chart audits, and survey readiness.
Support the Director of Nursing and office team with various administrative tasks related to patient care.
Qualifications
Active California LVN license in good standing (required).
At least 1 year of homehealth nursing experience
Strong understanding of Medicare guidelines and documentation (preferred).
Excellent communication and interpersonal skills — able to speak clearly with patients, families, and clinicians.
Strong computer and Perfect Noteefied EMR skills; proficiency with Microsoft Office (Word, Excel).
Organized, detail-oriented, and able to multitask in a busy environment.
Compensation & Benefits
Competitive hourly wage: $28 – $38/hr (DOE & qualifications).

Full-time, Monday – Friday (8:30 AM – 5:00 PM).
